South Umpqua High School Athletics is Now Using FinalForms!

We're excited to announce that South Umpqua High School Athletics is now using FinalForms, an online forms and data management system that makes athletic registration easier than ever for our families.

FinalForms allows parents/guardians to complete and electronically sign all required athletic participation forms online. Even better, your information is securely saved from season to season and year to year, so you won't have to re-enter the same information every time your student participates in a sport.

You can log in at any time to review or update your information, and you'll only need to re-sign forms when updates are made or when annual signatures are required.

What does this mean for you?

• All athletic paperwork will now be completed online through FinalForms.
• Parents/guardians of all students planning to participate in athletics at South Umpqua High School should create a FinalForms account and register their student(s).
• Returning families can simply update their information and select the sports their student plans to participate in.

If your student's current sports physical is already on file with the school, we'll upload it for you. Once your account is set up, you'll simply choose the sport(s) your student will be participating in for the upcoming season.

FinalForms also gives families the option to designate approved adults who may transport their student home from away contests.

Getting Started

1. Visit the South Umpqua School District FinalForms Portal.
2. Create your parent/guardian account.
3. Add your student(s).
4. Complete and electronically sign the required forms.

After creating your account, you'll receive a confirmation email. Be sure to open the email and click the confirmation link to complete your registration.
